Parish Registration

Welcome to our Parish! Before proceeding with Faith Formation registration, you must first register as a parishioner. Please complete the parish registration and return to begin the Formation registration process.

Parish ID is required to proceed with the formation registration. You will get an ID number only when you register as a parishioner.

Please follow the instructions and fill out the Parish Registration.

In the following pages, please provide information for each individual you would like to register for faith formation this year.

We also accept cash and checks. Payments can be dropped off at the parish office Mon- Fri 9:00 am - 3:00 pm, or mail check directly to:

Your family is considered fully registered when payment and required forms are received. Information on payment can be found at the end of this form. Thank you!

If, for any genuine reason, a family is unable to pay the registration fee or needs a payment plan, please contact the Formation Coordinator to make special arrangements.

OCIA (Order of Christian Initiation for Adults) program-Mondays from 6:30 PM – 8:00 PM

For Students from 7 yrs old - 18 yrs old.The OCIA (Order of Christian Initiation for Adults) is a faith formation process designed for students who are unbaptized and are seeking to learn more about the Catholic faith in preparation for receiving the Sacraments.

Adult Formation English – Wednesday 4:30 pm - 6:00 pm

Adult Formation English – Wednesday 7:00 pm - 8:00 pm

OCIA (Order of Christian Initiation for Adults) program-Mondays from 6:30 PM – 8:00 PM

For Students from 18 yrs old and older. The OCIA (Order of Christian Initiation for Adults) is a faith formation process designed for students who are unbaptized and are seeking to learn more about the Catholic faith in preparation for receiving the Sacraments.

In order to complete registration, you must provide:

  • A valid copy of individual’s baptismal certificate (Grades 1 - 12 ONLY)
  • Image Release Form (All Participants) : Click Here
  • These completed forms, online registration, and payment
  • Email all completed forms to Valeria Maldonado at vmaldonado@stjosephorlando.org . If you prefer, you may drop off the forms at the parish office.

Formation Fees

 

The pricing in the worksheet below is for Regular Registration: July 1 – July 31. If registering from May 13- June 30 this is Early Bird Registration. Take a $10 discount from each type of registration. You will receive a payment link the week of July 1. If registering from August 1 – August 22 this is Late Registration add $10 to each type of registration type.

First Reconciliation/First Communion Prep Fee and/or Confirmation Retreat Fee (These fees are paid in addition to the formation registration fee if you have a child in his/her second year of formation and is receiving a sacrament this year.)

There will be a mandatory parent meeting for elementary grade parents on August 27 from 6pm - 8pm, middle school parents on August 28 from 6pm - 8pm and high school parents on August 25 from 6pm - 8pm. All meetings will be at 6:00 pm in the social hall.
